Waiver Letter

This is a letter that will be included in your premed file. It will be added along with your composite letter so medical schools will know that you have voluntarily waived the right to see the contents of such letters. It is in your best interest to sign such a waiver, as it gives greater credibility to what is written about you.

Evaluator Request This is the form you give to someone who is going to write you a letter of evaluation. Choose these people wisely. Make sure it's going to be a good reference. If you get a poor one, it reflects back on you as someone who is a unable to read other people. Also, don't choose pastor, family member, or someone who will 100% of the time give you a GREAT opinion. Keep it to professional associations and profesors who know you pretty well.
Med School Application Hints Very helpful information on applying to medical school. Read it twice, and put it under your pillow at night.

Pre-Letter Checklist

Here is the big checklist is a summary of things you should do to prepare for application to medical/vet/dental schools, and important timelines.  The pre-letter checklist is for students who will be looking for a composite letter from Gordon's pre-HP committee. This is what we need so we can put your letter together.

VITA form

This two page form helps the pre-health advisor to write a more accurate and informative composite letter, and includes info about your work and volunteer history, etc. Make sure a copy of this is in your file at Gordon. You can email it to cstory@gordon.edu as an attachment (edit the MSWord version) or you can print the PDF version and mail it in.
List of Schools Use this form to tell me where and when to send your letter of evaluation, or better yet, just email us the list!
Timeline Agreement and HP Application The timeline form should be signed and turned in during the sophomore year. It is an agreement that you yourself will make a good faith attempt to stick to the expected timelines in your application process. The application form is for all health professions students so I can keep track of you, and know where you are headed (two sided).
